Description
Provides care and support for patients and their families that are facing end of life challenges.

Offers care and support for people with a life-limiting illness. Service includes caring for each client's physical, psychosocial, and spiritual needs. Treatment is offered for symptoms that have been hard to manage and need ongoing monitoring.

Once clients are stable, they are discharged to the most appropriate care setting. The average length of stay is 3 to 4 weeks, but may be longer.

  • patients must have agreed to a Goals of Care designation of M or C
  • have physical and psychosocial symptoms that cannot be managed anywhere else
  • must be 18 years of age or older.
