Psychiatrist follow-up, to bridge until connected with community resources.

The Psychiatric Transition Service is a bridging service offered by hospital psychiatrists. This service offers clients who:

  • have been seen in various inpatient or outpatient Addiction and Mental Health (AMH) services within the hospital
  • require additional psychiatric follow up and are not otherwise connected to psychiatric services

This is not an urgent or crisis service.
