- Provides a 28 bed capacity program to support individuals experiencing homelessness to convalesce in the community by providing health services to individuals that require medical supports, but are not ill enough to be admitted to acute care.
Service provides:
- a nurse-led health and social supports program for individuals experiencing homelessness with medical needs that require community-level care and are not ill enough to be admitted to hospital but are too sick to be in a regular shelter or out on the street
- medical site with 28 bed capacity
Services may include:
- wound care
- medication management
- care for pneumonia
- social work services to connect individuals to a variety of supports, such as housing, income support, job search, and addiction recovery-oriented services
