Transition to Adulthood Program

Service provides:

• help to prepare young adults in and from care to live independently, pursue education, and build their careers

Includes:

• 4 different pathways to help transition to adulthood; the Admissions Team collaborates with young adults to determine which pathway they want based on their strengths, goals, hopes, and dreams
• access to social and emotional transitional supports, mentoring opportunities, life skill development, mental health and addiction supports, help finding work, and cultural connections
• access to standardized monthly financial supports as well as funding towards accommodation expenses

The 4 pathways include:

• Adult Support Services Pathway - connects to lifelong adult supports like Assured Income for the Severely Handicapped (AISH) and Persons with Developmental Disabilities (PDD); includes support from a Transition to Adulthood Program (TAP) practitioner while also accessing the adult support services that will help the transition to independence goals
• Stability Pathway - young adults are supported to feel stable and receive help based on individual needs; TAP clients are supported to connect with mental, physical, or addiction supports while continuing to be supported with housing and monthly costs so they can stabilize
• Employment and Career Pathway - access supports that will help with entering the workforce and reaching employment goals; this might look like work search, interview preparation, resume building, and other career planning tools
• Education (Advancing Futures) Pathway - young adults can earn a degree or diploma, learn a trade, receive a certificate, or obtain a high school diploma if it's a requirement for entry into a post-secondary program

• young adults from 18 to the day before their 24th birthday
• who, at the time of their 18th birthday, must have had an open file with Children and Family Services under: Permanent Guardianship Order (PGO), Permanent Guardianship Agreement (PGA), Temporary Guardianship Order (TGO), Custody Agreement with Youth (CAY), or Enhancement Agreement with Youth (EAY)
