- Provides personalized support services to individuals who live in their own home and do not require overnight support who may experience a crisis situation or other emergent needs that necessitate immediate support.
Service provides:
- personalized support services to meet individual needs and circumstances of individuals who may experience any number of life challenges or daily living skills needs that prevent them from successfully achieving their goals
Includes:
- community integration to enhance and sustain an individual's quality of life
- flexible hours of support dependent on individual direction
- assistance in identified daily living skills domains and accessing essential community resources as well as transitional support for significant changes in lifestyle, personal circumstances, or needs
- Eligibility
- Yes, Service targeted to:
- 18 years and older
- with a documented developmental disability
- who are reasonably independent and medically stable
- and are eligible for funding through Persons with Developmental Disabilities (PDD)
