Description
Wellness Integrated Support Homes (WISH) is a long-term supportive housing program for adults experiencing addiction and mental health issues.

Offers living spaces for adults who need 24 / 7 access to help with daily activities.

Services include:

  • help with daily activities, home making, and medicines
  • monitoring health
  • access to recreation activities
  • scheduling professional services

Living spaces are usually small group homes where 5 to 8 people live together and get support from a Health Care Aid. Residents have private bedrooms and share all other common areas of the home. Basic bedroom furnishings are provided.

Each resident has a Case Manager who assesses health care needs on an ongoing basis, co-ordinates and schedule access to health care services.

Eligibility
Yes, Service is provided to adults with addictions and mental health concerns who are living in conracted personal care homes and require additional supports to live successfully in the community.
