The Geriatric Mental Health Consulting Service - Transitions Team provides comprehensive mental health assessment and psychiatry consultation to clients over the age of 65 in subacute, Carewest Transitional Units.

These transitional units were developed to support clients from acute care in their rehabilitation prior to their discharge to the most appropriate living situation.

This includes:

  • Carewest Sarcee - Regional Community Transition Program (RCTP)
  • Carewest Fanning - Neuro-rehab units
  • Carewest GlenmoreMusculoskeletal Rehab unit/enhanced care unit (medically complex)

Types of services delivered:

  • mental health assessment and consultation
  • psychiatry consultation (includes medication review/capacity assessment)
  • support in discharge planning

Length of invovlement is typically while the client is admitted to the Transitional Units.
