- Physiotherapy assessment, treatment, and education for rehabilitation focused on improving physical function after orthopedic surgery or hospital admission within the last 3 months.
Participation in this program requires the patient:
- is motivated and able to participate in an active rehab program, including a home exercise program
- has identified functional rehabilitation goals and is motivated to achieve them.
- is able to attend scheduled appointments in the outpatient department
- is their own decision maker and able to make decisions related to assessment and treatment or the designated decision maker (parent / guardian) is available to provide informed consent for ongoing assessment / treatment decisions.
