Description
Provides a Housing First program for chronically homeless people to find safe and stable housing.

Service offers:

  • 12 months of assistance and support in finding and setting up a market apartment in the client's neighbourhood of choice

Includes:

  • developing a service plan with the goal of increasing housing stability
  • supports to learn and explore the community
  • connecting to new community resources
  • regular weekly home visits
  • monthly landlord check-ins

  • 18 years and older
  • chronically homeless individuals
  • episodic homeless individuals

Service Access: Professional Referral Required, To access service:
  • referral from Homeward Trust Co-ordinated Access required
  • visit a Homeward Trust co-ordinated access site to enroll to waitlist
  • assessment is required to determine eligibility for the program
