Provides housing wayfinding and follow up support for chronically homeless Indigenous people in Edmonton.

Service offers

  • access to a Housing Outreach Worker and follow up support for chronically homeless Indigenous people in Edmonton

Assistance includes:

  • searching for and identifying housing
  • building relationships with landlords, including mediating conflicts
  • applying for and managing housing allowances
  • setting up apartment (furniture, household items, groceries)
  • accessing resources and supports within the community
  • developing a personal network of support

Eligibility
Yes, Service targeted to:
  • Individuals receiving income
  • who are chronically homeless (4 episodes of homelessness within a 3 year period OR continuous homelessness within 1 calendar year)
Service Prioritization Decision Assistance Tool (SPDAT) score also required.
