Description
Service provides housing for young adults who were previously experiencing chronic or episodic homelessness and who may be subject to substance addictions.

Service offers:

  • a safe living environment
  • support to help tenants move from being at risk for experiencing homelessness to living independently
  • identification of strengths and goals
  • support with developing a plan to attain goals
  • referral to appropriate personal, professional and community supports and services
  • building one-on-one and community relationships
  • housing first, trauma-informed care and harm reduction models of care
  • staff on-site 24/7

Service Access: Professional Referral Required, To access service:
  • seek referral from SORCe
Triveri House is a part of CAA (Coordinated Access & Assessment). Meaning that a NSQ (Needs & Service Questionnaire) is required to access the program. The Needs & Service Questionnaire can be completed at the SORCe.
