- Keeping older adults with complex health issues; healthy, at home and out of the hospital.
Provides services for older adults who have complex health issues and want to remain living in their own homes. The goal of the program is to keep clients healthy, at home, and out of the hospital system. Services may include:
- medical monitoring and treatment by physicians and nurses
- medications dispensed by the pharmacy
- physical and occupational therapy
- social work (and Pastoral Care Services at a Good Samaritan location)
- recreation and social activities
- personal assistance with daily tasks, as needed
- health and wellness education

- Yes, Standard CHOICE criteria plus the following for the Special Care Program:
- established diagnosis of dementia
- requires programming adapted to their needs in a secure and supportive environment
- participant requires a smaller secure environment to support the significant nature of their dementia or active wandering

There is a monthly fee for CHOICE.
Clients also pay the co-payment for medications not covered by their Blue Cross or other insurance up to $125.76 per month. Over the counter medications are not included in the $100 co-payment.
Fee Reduction: Yes,This program fee may waived for those on full benefit assistance for Alberta Aides for Daily Living (AADL), receiving Senior’s Benefit assistance, and/or under 65 years of age and on AISH.
