- Keeping older adults with complex health issues; healthy, at home, and out of the hospital.
Provides services for older adults who have complex health issues and want to remain living in their own homes. The goal of the program is to keep clients healthy, at home, and out of the hospital system. Services may include:
- medical monitoring and treatment by physicians and nurses
- medications dispensed by the CHOICE pharmacy
- physical and occupational therapy
- geriatric psychiatry
- social work (and Pastoral Care Services at a Good Samaritan location)
- recreation and social activities
- personal assistance with daily tasks, as needed
- health and wellness education

- Yes, Standard CHOICE criteria plus:
- over the age of 60, with primary diagnosis of mental illness diagnosed by a psychiatrist
- excludes those with a primary diagnosis of dementia with psychiatric symptoms
- require the ongoing support of professional staff with specialized skill in working with mental illness
- cannot be maintained with other available community supports or within a regular CHOICE program

There is a monthly fee for CHOICE. Clients will be receiving their medication from the CHOICE pharmacy and are required to pay the co-payment portion for their drugs not covered by the Alberta Blue Cross plan or other drug plans up to maximum of $125.76 per month.
Fee Reduction: Yes,This fee is waived for those on full benefit assistance for Alberta Aides for Daily Living (AADL), receiving Senior’s Benefit assistance, and/or under 65 years of age and on AISH.
