- CHOICE South Restorative is a short-term, intensive rehabilitation program that helps clients improve or maintain their health and independence. This program is guided by a "work with you" approach, where clients are expected to be active participants in setting and working toward functional goals.
Clients will come to the CHOICE Centre Road several times a week. Based on their identified goals and care plan, they can receive the following:
- medical monitoring and treatment
- rehabilitation or restorative therapy
- therapeutic recreation, cognitive and social programs
- personal assistance with daily tasks like going to the washroom (as needed)
- health and wellness education
- meals and snacks

- Yes, Primarily focused on age 55+; Requiring a coordinated approach to care because of complex medical, social, functional and / or psychological needs;
- medically stable (i.e. not at immediate risk of hospitalization);
- able to tolerate attending the program for 4 to 8 hours
- able to physically attend the program, potentially multiple days/week.
- willingness, motivation and ability to participate in Program activities, including goal setting and / or working to achieve goals
