Supportive Living

Service provides:

• a home-like apartment environment

Includes:

• medical and social supports to seniors and individuals with a disability requiring continuing care
• health care aides onsite 24 hours a day to provide support including personal care and medication assistance
• case managers to coordinate care, including rehabilitation and nursing
• a site designated physician or nurse practioner, if resident does not already have a physician/nurse practitioner

• 65 years and older
• living with physical disability, mental health diagnoses, or mild dementia
• not a risk to themselves or others, no risk of wandering
• medically or physically stable but require scheduled or unscheduled supports
• ability to move independently or with some assistance
• ability to eat with minimal assistance and have minimal dietary or food texture modifications
• ability to use call system if help is needed

• assessment required through Alberta Health Services
