- residing in Airdrie, Balzac, Carstairs, Crossfield, Irricana, Kathryn, Keoma, and Madden
- who have a life-limiting illness

- professional referral required confirming life-limiting illness from a physician, nurse, or Alberta Health Services palliative care worker
