- Provides a systems navigation service and outreach for at-risk youth.
Service offers:
- systems navigation for youth, including those with complex needs
Includes:
- assistance getting connected with permanent housing, mental wellness, financial supports, and other resources needed to fully participate in community life
- support from youth outreach workers for community-based youth that are at risk of becoming homeless due to family conflict, as well as those currently homeless or staying in the Inn Between
- appropriate housing / re-housing of youth
- support to the family to promote family reunification
- "Common Ground" Parent / Youth Conflict Resolution - 1-to-1 support for youth and families in order to prevent youth homelessness
