- phone Service Canada to request information and assist in completing applications
- visit website to apply online ( (requires a My Service Canada Account)
- visit website to apply using a paper application
- include certified true copies of the required documentation
- mail the form or drop it off at a Service Canada office
- include both the deceased person's Social Insurance Number and your own on all documents before sending them to Service Canada
