- Eligibility
- Yes, Service targeted to individuals who are:
- registered as a home care client
- assessed as requiring a type of wellness service
- living in the community or in the process of being discharged home
- in contact with a physician in the community
- motivated to become involved and participate in the program
- able to function in a group and not demonstrate behaviour harmful to themselves or others
- do not requre 1-to-1 supervision
- and are able to attend on a regular basis
