Crossroads Day Program

Service provides:

• support and assistance for individuals in the community to maintain their independent living situation.

Includes:

• a day program with a variety of social programs
• family education and support
• recreation activities, including exercise, discussions, table games, baking and cooking, crafts, music and singing, hand bells, video games and mind joggers
• socialization
• special diets

• registered as a home care client
• assessed as requiring a type of wellness service
• living in the community or in the process of being discharged home
• in contact with a physician in the community
• motivated to become involved and participate in the program
• able to function in a group and not demonstrate behaviour harmful to themselves or others
• do not requre 1-to-1 supervision
• and are able to attend on a regular basis

There is no fee for the program; there is a $10 fee for the optional lunch and snacks.
