Home Support Program

Service provides:

• support to help families stay together during times of crisis such as long or short-term illness, injury, or hospitalization
• care plans and assistance for seniors and people with disabilities so that they are able to remain living in their own homes (the care plan will be reviewed regularly to make sure it remains current to the needs of the client)
• indoor services only such as light housekeeping and meal preparation (not a professional cleaning service)
• referrals to the appropriate agency if they are not able to assist a specific individual

The length of time a client remains on the program will be determined by health condition and family support status.
