Bridge to Care


Description
Provides support and healthcare services for people with physical and mental disabilities.

Service provides:

  • support and healthcare services for adults and children with physical and mental disabilities
  • behavioural support and care for people with medical needs

Includes:

  • Autism Care - supporting individuals with autism and Asperger syndrome from their own homes, in residential care settings, and in the community
  • Community Access Programs - individualized vocational programs designed for persons with developmental disabilities, including persons with complex behaviours whose needs have not been met through traditional day programs
  • Family Support Program - full time and relief placement in private host homes for children with developmental and / or physical disabilities or sensory impairment
  • Fetal Alcohol Spectrum Disorder (FASD) Program - advocacy and support services to adults and youth 13 years and older diagnosed with FASD
  • Independent Living Program - skill development opportunities, service co-ordination, support, guidance, and crisis intervention to adults with developmental disabilities and / or mental illness who live on their own in the community
  • Outreach Program - room and board, supervision, training and skill development opportunities in private community homes for adults and / or seniors with developmental disabilities and / or mental illness
  • Residential Program - support and healthcare services to individuals with physical and mental disabilities that includes accommodation and supervision and assistance with personal hygiene and grooming
  • Respite Support - 24/7 respite support to families of individuals with special needs
  • Therapeutic Services - support with physiotherapy (focus on gross motor skills, endurance, and fitness), occupational therapy (focus on upper extremity functions, fine motor skills, visual motor functions, and sensory processing skills) and speech and language therapy (focus on communication and cognition by enhancing verbal or communication expressive language skills)
