Description
Provides support to people of all ages living with advanced illness as well as their family members and close friends. Assisting people to stay at home as long as they are able.

Service offers:

  • a team of professionals providing individual and family counselling during illness and after a death (counselling can occur at the office, in homes, at hospitals / hospices or in the community)
  • peer support programs during illness or after a death
  • Dying to Know Education Program for anyone interested in learning more about palliative care and grief
