Description

A comprehensive palliative care service, including a community-based hospice, for children with life limiting illnesses and their families. The pediatric palliative care service provides an extra layer of support to the child and family, along with their primary teams, throughout the child's illness trajectory. Grief and bereavement support for parents and siblings is also provided.

This service provides support to children with life limiting illnesses and their families, including:

an interdisciplinary team offering support around new diagnoses of life threatening conditions, difficult decision-making, anticipatory grief support and collaboration in comprehensive care planning
consultative support for care of the dying child in hospital, hospice or community
Rotary Flames House, a palliative care facility which offers respite, symptom management and end of life care in a home-like setting
outpatient programming for children and their siblings
grief support for bereaved parents of children 17 years and younger

Key Provider: chaplains, child life therapists, doctors, grief support counsellors, health care aides (HCAs), registered nurses (RNs), social workers

Eligibility

Yes, Children 17 or younger, with serious illnesses or chronic complex conditions where progression to death, sometime in childhood, could be expected. Pregnant women who are carrying a fetus with a diagnosis of a serious condition.
