Description
Offers support to individuals with a spinal cord injury or other physical disability during transitions from hospital to community, to home, to employment, or to aging with a disability.

Service provides:

  • support to individuals with a spinal cord injury or other physical disability as determined by the unique needs of the individual

May include:

  • support to become an effective self-advocate to obtain accessible housing, employment, education, medical professionals, personal care, and sustainable income

  • assistance with the completion of application forms, advocacy to Alberta Income Supports, and referrals to more service providers
