- An interdisciplinary home care team that provides geriatric consultation service to home bound, home care clients in the City of Calgary.
The Geriatric Consult Team completes in-home comprehensive geriatric assessments of home bound, home care clients in the City of Calgary.
Our clients are assessed for complex medical decline, functional decline, multifactorial or unexplained falls, medication concerns, unexplained cognitive decline, or other symptoms that could lead to falls or an emergency room visit.
Key Provider: nurse practitioners (NPs), occupational therapists (OTs), pharmacists, physical therapists (PTs), registered nurses (RNs), and therapy assistants (TAs). - Eligibility
- Yes, Adults 65 years and older.

Client must be a Home Care client to access this service.
