Canada Pension Plan Disability Benefit

Service offers:

• a monthly taxable benefit for individuals with a severe and prolonged disability that have made contributions to the Canada Pension Plan

• individuals that have a mental or physical disability that regularly stops them from doing any type of substantially gainful work
• individuals that have a disability that is long-term and of indefinite duration, or is likely to result in death
• individuals age 65 and under
• individuals contributed enough to the Canada Pension Plan

• submit a medical form 
• by phone, inquiries can be made about application status or the monthly payment amount and date
• visit Service Canada Centres to submit supporting documents, or to use the telephone interpretation service
• visit website to find a Service Canada Centre

Individuals must work with a doctor or nurse practitioner to submit one of the following forms that applies to their situation:

• Medical report form for non-terminal illness
• Medical report form for terminal-illness only

It can take up to 120 days (4 months) from the date of application submission, including required documents, to receive a decision.

If more than 120 days (4 months) have elapsed without communication, please reach out to Service Canada for further assistance.

A letter will be mailed with a decision.
