Description
The Stroke Early Supported Discharge Program (SESD) provides client-focused, virtual or in-home therapy for adults with stroke who are referred from hospital upon discharge.

This program can be done at home or through virtual sessions. People in this program get up to 6 weeks of therapy. It is for those with mild to moderate problems who need therapy at home, or for those who cannot go to a rehabilitation center. People in the program help decide their therapy goals.
