Ensuring complex medical patients have a safe transition from hospital to home. Access for patients to a Pharmacist post discharge for question and concerns. The unit clerk ensures all diagnostics and outside referrals are completed as ordered.
At home telephone consult 2-7 days from discharge to assess the patient and to address questions and concerns or ongoing medical issues.
