- Works collaboratively with children, youth, and families to help build individual support networks in their own communities.
Service offers:
- help with reintegrating or maintaining children with their families
- preparing youth for life on their own as they age out of care
- a multidisciplinary team that consists of family specialists, case workers, the family, and any identified support members to the family
May include:
- life skills coaching about topics such as healthy relationships, parenting skills, conflict resolution
- linkage to external community resources
- supported visits
- goal-oriented drives
- advocacy and support
- support groups
- and more
All services are directly tied to the goals of reunification or maintaining the family unit. This is a contracted Family Preservation Program that is strength based and family centered.
