- Provides in-home therapy to stroke survivors who have been referred from the hospital after they have been discharged.
The Stroke Early Supported Discharge (ESD) team helps clients return home from hospital earlier following a mild to moderate stroke. The (ESD) team provides a short-term, client-driven, multidisciplinary rehabilitation program in the client's home and community environment, helping to ease the transition from hospital to home to community.
