Connects community participants with affordable housing and community supports using a Housing First model.

Service offers:

  • affordable housing and community supports in agency-owned-and-operated housing facilities

Includes:

  • Westwood Manor - a 20-unit permanent supportive housing initiative that serves men and women with a history of chronic homelessness through a harm reduction model; residents can access support staff on site 24 / 7
  • Open Door Manor - a 16-unit, semi-supportive apartment facility to participants who are reintegrating into society after incarceration
  • Prairie Manor - 85 affordable housing units with wrap-around supports
Eligibility
Yes, Service targeted to:
  • 18 years and older
  • who have previously been homeless or are reintegrating into society after incarceration
There may be additional eligibility criteria depending on the housing building.

Service Access: By Appointment, To access service:
  • call to speak to wellness advocate or for more information
Referral process in place and individuals assigned to a wellness advocate / housing support worker. The wellness advocate / housing support worker team partners with Homeward Trust.
