- assistance to individuals with a severe and persistent mental illness to live independently and interdependently in the community
Includes:
- helping individuals identify needs and interests and then developing a plan to address these areas
- teaching individuals how to advocate for themselves to remove barriers
- co-ordinating services with other agencies to provide support and assistance to promote social, physical, and emotional well-being while fostering self-determination
