- The ED Transition Clinic is available for patients treated and discharged from the Strathcona Community Hospital Emergency Department. Patients may be referred to the clinic for short term management, until the patient can be seen by their primary care physician.
The ED Transition Clinic is a Nurse Practitioner led clinic. Offered services include:
- wound care
- medication management
- transitional care

teaching for any condition needing short-term follow-up to ensure an effective treatment plan is implemented and evaluated
Care is offered as a transition until patient can be seen by their primary care physician (PCP). If no PCP has been identified, the team will assist the patient in connecting with one.
