Description
Provides a program for youth and their families to support the young person's transition into adulthood.

Service offers:

  • support to prepare for adult life through one on one counselling
  • group discussions
  • workshops with guest speakers
  • a comprehensive list of topics for youth, including postsecondary education, health care, resources, and funding
Eligibility

  • individuals age 16 to 25
  • individuals with cerebral palsy or other disability
