- interview required with a Child Support Worker
- birth and marriage certificates
- previous agreements or court orders, if available
- address, telephone number, and a recent picture of your child's other parent
- record of support payments already made, if applicable
- confirmation of your income including your last 3 years of Income Tax Notices of Assessment and 3 recent paystubs, if applicable
- confirmation of children's expenses including childcare receipts, music lessons, tutoring, and athletic activities, if applicable
