Home Care helps people who live at home and need support such as:

  • Care after surgery
  • Long-term care
  • Palliative care
  • Respite services

A case manager, who is usually a nurse, will meet with the clients and help decide what services will best support them.

Key Provider: Licensed Practical Nurses (LPNs), Occupational Therapists (OTs), Registered Nurses (RNs)

Service Access: By Appointment, Referrals to Home Care are placed though Continuing Care Access (CCA) at 1-855-371-4122. The CCA team can be contacted by you, family, friends, healthcare providers or community agencies to request services.

The Continuing Care Access team has a 24/7 voicemail option and operates the following hours:

Monday to Friday 8:15 am to 8:00 pm,

Saturday, Sunday and Statutory Holidays 8:15 am to 4:30 pm.
