Service offers:
- supportive mentor-style, home-based placements for youth who need more intensive support, guidance, and assistance than is available in existing programs
Includes:
- support for youth who will be transferring to the adult support systems, such as Assured Income for the Severely Handicapped (AISH) and / or Persons with Developmental Disabilities (PDD)
- a group living transition home
- social relationship skills
- rebuilding ties with families using Support Circle Process
- self-development skills
- personal wellness and health

- 16 to 23 years with Children's Services status or Family Support for Children with Disabilities (FSCD) status
