- The Program offers home-based and virtual support to families with children that have developmental delays or disabilities, aged 0-5 years of age.
The program offers:
- information about development, behavior, play and parenting
- connections to community resources and services
- support to your family setting goals and planning for the future
- collaboration with your child's health team
- services that are provided in various ways; in your home, in the community, virtual appointments or phone consultations

- Yes, Children between 0-5 years old are eligible if:
- you live within the Central Zone
- you are wondering if your child is meeting developmental milestones
- your child is experiencing or is at increased risk for developmental delays
- the most appropriate service based on your child and family’s needs
- Application Process
- Service Access: Professional Referral Required, Healthcare providers should consult the Alberta Referral Directory for service referral information.
Parents can self-refer. Healthcare providers, community agencies, pediatric rehabilitation services, doctors, and public health nurses can also refer.
