Description
Provides supportive services to seniors with complex medical needs and provide social support to help them remain in their own homes.

Service provides:

  • support to seniors with complex medical needs, who might otherwise be eligible for admission to a continuing care centre, so that they may continue living at home

Includes:

  • physician and nursing care
  • health and wellness education
  • rehabilitation services
  • pastoral care
  • social work services
  • medications and pharmacist consultants
  • meals and dietary consultation
  • transportation may be available to and from the program at no cost
  • recreation and socialization
  • home support (based on assessed need)
  • access to overnight respite care (if appropriate)

Affiliation: Partnered with Alberta Health Services.
Key Provider: chaplains, dietitians, nurses, physicians, recreational therapists, rehabilitation therapists, social workers
