Systems Navigation and Outreach

Service provides:

• drop-in services and home visits

Drop-in services (office) include:

• 1-on-1 confidential information
• holistic assessment of needs
• referrals
• assistance in understanding and filling out government forms
• follow-up with government officials
• linking to food security programs
• advocacy
• linking to transportation services
• and basic education and advice on how to address elder abuse

Home visits by the senior systems navigator (social worker) include:

• assessing needs
• making subsequent referrals to community-based services and supports
• helping clients connect with government officials
• and providing case updates to participants with mobility challenges who can't leave their home

• St. Albert residents
• who are 50 years and older
• or their caregivers

• call, visit website and create online membership account, or call to register for information sessions
• call reception or e-mail for help or a friendly phone call and leave a detailed message of specific needs

Annual Individual $110.00 (pro-rated fees offered)
