- Provides occupational therapy assessment and short term services for adults living with chronic health conditions to maximize participation, engagement and safety in their home and communities. Occupational Therapy assessments are completed in the client's home.
Offers assessment and intervention focusing on:
- activities of daily living: mobility, bathing, dressing
- maintaining instrumental acivities of daily living: meal prep, access to community, driving (No on-road testing)
- home safety and adaptations
- fall risk and prevention
- lower leg edema management for home bound clients
- chronic disease management
