Based on assessed unmet needs, Children's HC provides case management along with health and developmental services and supports to children / families in their homes or community setting including:
- transition from hospital to home
- acute health needs
- long term health needs
- end of life support and care

- Yes, Program serves children and youth, from birth to 18 years of age if eligible for Home Care (based on the Coordinated HC Program Regulations).
