Provides a referral service for clinicians who need to refer people to a geriatric clinic for issues such as:
- mobility (moving around inside and outside their homes)
- falls risk
- medication
- incontinence (controlling urination and bowel movements)
- cognitive decline (thinking and remembering)
This service schedules patients into the following clinics:
- Glenrose Rehabilitation Hospital Geriatric Assessment Clinic
- University Hospital Seniors Health Clinic
- Grey Nuns Community Hospital Geriatric Outpatient Clinic
- Misericordia Community Hospital Seniors Clinic
- Strathcona Community Hospital
- Good Samaritan Seniors Clinic
