Description
The role of the Social Work department is to consult with other healthcare professionals to address a patient's care needs and provide support to help them deal with illness or injury.

Services include:

  • Conducting assessments to inform care plans
  • Educating patients and families on health, treatment, and resources
  • Collaborating with healthcare teams to address barriers and psychosocial factors
  • Offering counseling and therapy for coping, mental health, and addiction
  • Referring to community and government services for ongoing care
  • Facilitating transitions between care settings
  • Engaging in research and advocating for improved healthcare access and quality
  • Advocating for patients, families, and communities when quality healthcare is unavailable or inaccessible
