Provides short-term follow-up for recently discharged clients of the Rockyview Hospital and Carewest Glenmore Mental Health Recovery and Rehabilitation Unit.

Provides short-term follow-up for recently discharged clients of the Rockyview Hospital and Carewest Glenmore Mental Health Recovery and Rehabilitation Unit. The goal of the follow-up is to help clients enter back into the community.

Services provided include:

  • case management for up to 90 days
  • medicine monitoring
  • community services referrals
  • supportive counselling
  • caregiver support
  • consulting referring psychiatrists
