- Description
- Provides urgent, short-term social work intervention for older adults living in the community requiring immediate basic needs support, facing barriers in accessing formal supports and services, hesitating to reach out for help, or being identified by community members as living at risk.
Service offers:
- conducting holistic needs assessments
- assisting in accessing basic needs such as clothing, food, and medication
- stabilizing housing or facilitating access to alternate independent senior community housing (not a housing provider and no emergency housing)
- connecting individuals to long-term community support to enhance well-being and prevent crises
- accepting referrals for isolated older adults hesitant to seek support
- conducting in-person visits to offer assistance and services
- support with resource and system navigation for families of reluctant older adults
