Assess seniors with chronic and complex health issues, with the goal of reducing hospital admissions and improving quality of life for our clients and their caregivers.
Key Provider: geriatricians, licensed practical nurses (LPNs), occupational therapists, registered nurses (RNs) - Eligibility
- Yes, Age 65 and over and living in Alberta, or younger than age 65 who suffer with a geriatric syndrome i.e. early onset dementia.

Referrals are received from family doctors, Home Care Nurses, family members via fax.
