- Provides services to adults with a physical and / or neurological functional loss and who would benefit from 2 or more rehabilitation therapies. Serves all of Central Zone.
The service helps clients and their families and / or caregivers to reach the best physical, emotional, thinking, and spiritual health possible. This program helps clients to function as independently as possible and within their own community.
Includes those with:
- stroke
- neurological conditions
- orthopaedic conditions
- post-traumatic conditions.
Patients participate in daily intensive rehabilitation therapy sessions based on their needs. Patients on Unit 35 require 24-hour nursing care in addition to their rehabilitation needs, but are medically stable.
Key Provider: dietitians, doctors, health care aides (HCAs), licensed practical nurses (LPNs), occupational therapists (OTs), pharmacists, physical therapists (PTs), recreational therapists, registered nurses (RNs), social workers (SWs), speech-language pathologists (SLPs), therapy assistants (TAs) - Eligibility
- Yes, Clients age 18 and over with functional loss related to stroke, neurological, orthopaedic, or post-traumatic conditions who require 24-hour nursing care and the support of at least two allied health disciplines.
Acute spinal cord and traumatic brain injuries are exclusion criteria as are patients who are moving to placement (supportive living, long-term care, etc.). Single discipline referrals are not accepted. - Application Process
