Description
Offers recovery-focused housing opportunities for people experiencing mental illnesses.

Service provides:

  • 16 transitional and 8 long-term residential beds for individuals to develop and / or maintain independent living skills such as medication management, symptom management, nutrition and meal preparation, household maintenance, and money management skill
  • a team of multidisciplinary staff who work with the client to assess needs, develop an individual service plan for their stay, and provide the guidance and direction needed to help clients recover and re-establish themselves in the community
  • progams and supports for clients

Includes:

  • permanent supportive housing

  • transitional / stabilization housing

  • respite housing

  • assertive intensive case management

  • independent living skills support

  • housing first

  • community treatment order

Staff support is available 24 hours daily. Lengths of stay in the transitional program vary from a few months up to a year. Long-term clients stay as long as the program is able to meet their needs.

Eligibility
Yes, Service targeted to:
  • 18 to 64 years
  • with a diagnosed severe and / or persistent mental illness
  • who are willing to participate in programs
  • and are able to function in an open residential setting
The program is unable to accomodate:
  • individuals whose addiction interferes with their day to day function and / or others comfort and safety
  • individuals assessed as high suicide risk
  • physical health conditions requiring ongoing medical support
  • primary diagnosis of developmental disability
  • high risk behaviours putting themselves and / or others at risk
